All documents must be duly notarised and attached to the application form.

Company/Partnership Information:

• Import-Export Code (IEC): Issued by the DGFT
• GST Certificate:
• Partnership Deed/ Memorandum of Article of Association: (If applicable)  

Partner/Director Information:

• ID Proof: Of Proprietor & all Partners/ all Directors (Whichever is applicable)
• Address Proof: Copy of Electricity Bill/ Telephone Bill/ Bank Statement

Storage Facility Information:

• Storage Plan Layout: Approved by a civil engineer (If applicable)

Waste Import Details:

• Waste Description (Break-up): Under Part-D of Schedule III with approximate quantity per annum in MTPA (to be mentioned in Form 7)
• Purchase Order/Performa Invoice: Copy

Authorized Actual User Information:

• Details of Authorized Actual User(s): Name, address, and PCB ID/Copy of CCA

Notarized Memorandum of Understanding (MOU): On Rs. 100 stamp paper for selling of imported scraps to authorized actual user
